Requires individual accident or health insurance policies issued on or after January 1, 2015 to evaluate and review coverage for orthotics, prosthetics, orthoses, and prostheses by comparing to federal Medicare standards under 42 U.S.C. sections 1395k, 1395l, and 1395m.

Permits insurers to require prior authorization for orthotics and prosthetics in the same manner as other covered benefits and to limit coverage to the most appropriate model as determined by the treating physician.

Allows repair and replacement of orthoses or prostheses subject to copayments and deductibles unless caused by misuse or loss, and permits insurers to require that services be rendered by certified orthotists or prosthetists and provided by vendors.

Applies the same requirements to group, blanket, and franchise accident or health insurance policies and health maintenance contracts issued on or after January 1, 2015.

Excludes hospital confinement indemnity, disability income, accident only, long-term care, Medicare supplement, limited benefit health, specified disease indemnity, and other limited benefit policies from these requirements.
