Designates Medicaid prepaid behavioral health plans and managing entities as responsible for coordinating services for mental health residents in assisted living facilities with limited mental health licenses, with required community living support plans and case manager face-to-face interaction documentation.

Changes the threshold for requiring a limited mental health license from three or more mental health residents to one or more mental health residents in assisted living facilities.

Establishes specific administrative fines based on facility bed capacity: $7,500 (fewer than 100 beds) or $11,250 (100+ beds) for Class I violations; $3,000 or $4,500 for Class II violations; $750 or $1,125 for Class III violations; and $150 or $225 for Class IV violations.

Creates "flexible bed" licenses allowing continuing care facilities and retirement communities to provide assisted living services to up to 15 percent of independent living residents, allowing residents to age in place without transferring to separate facilities.

Requires the Agency for Health Care Administration to implement a rating system for assisted living facilities by November 1, 2014, and create accessible website content including facility information, violations, corrective actions, and a monitored public comment page by January 1, 2015.
