Requires motor vehicle registration and renewal application forms to include an option for voluntary contributions of $1 or more per applicant to End Breast Cancer, with funds distributed to the Florida Breast Cancer Coalition Research Foundation, Inc. for breast cancer research and education.

Adds a new option (t) to driver license and identification card application forms allowing voluntary contributions of $1 or more per applicant to End Breast Cancer, distributed to the Florida Breast Cancer Coalition Research Foundation, Inc.

Contributions are classified as not income of a revenue nature for purposes of applying the service charge under Florida Statutes section 215.20.

Applicants on driver license and identification card forms will have multiple voluntary contribution options for various health, disability, veteran, and social causes alongside the new breast cancer option.

Effective date is July 1, 2015.
