• Defines "supportive services" as services addressing client needs including housing, food assistance, education, employment, and social support offered by governmental, quasi-governmental, private sector, or community-based organizations.

• Requires county intake agencies to use an electronic shared information platform to provide appropriate community referrals and report on successful service delivery rates to clients.

• Expands the continuity of care management system to reduce client admissions to state treatment facilities, prisons, jails, and reliance on emergency medical services and homeless assistance services.

• Directs the Department of Children and Families to include performance measures and data on supportive services delivery in contracts with service providers, available in real-time electronic format.

• Establishes as legislative intent the creation of a shared platform for service delivery providers to access real-time referral information regarding appropriate supportive services for clients in their communities.

• Effective date: July 1, 2015.
