Requires community-based care lead agencies to contact adoptive families one year after adoption finalization to offer post-adoption services and document contact attempts and services provided.

Establishes an adoption incentive program requiring the Department of Children and Families to negotiate outcome-based agreements with lead agencies and providers to increase adoptions and award performance-based incentive payments.

Creates adoption benefits for qualifying state employees who adopt children from the child welfare system, providing $10,000 for special needs children and $5,000 for other children, with prorated amounts for part-time employees.

Requires licensed child-placing agencies conducting intercountry adoptions to be accredited by the U.S. Department of State and maintain records including family medical history, translated documents, citizenship documents, and supervisory reports.

Modifies educational placement requirements to prioritize children remaining in their school of origin when in out-of-home care and prohibits prejudice against caregivers who choose homeschooling; appropriates $6.5 million for adoption incentive program and $3.4 million for employee adoption benefits in fiscal year 2015-2016.
