Adds voluntary contribution option for "End Breast Cancer" to driver license and identification card application forms, with contributions of $1 or more per applicant distributed to the Florida Breast Cancer Coalition Research Foundation, Inc.

Specifies that contributions received under these provisions are not considered income of a revenue nature for purposes of applying service charges under Florida Statute 215.20.

Effective date is July 1, 2015.
