Requires hospitals and health care practitioners to offer hepatitis C screening tests to veterans born between January 1, 1945, and December 31, 1965, unless the veteran is in a life-threatening emergency, has previously been tested, or lacks capacity to consent.

Applies to veterans receiving inpatient general hospital care, primary care services in hospital settings, or primary care from licensed physicians, dentists, or advanced registered nurse practitioners.

Mandates that positive screening test results be forwarded to the veteran's primary care practitioner, who must provide counseling and followup care including a hepatitis C diagnostic test.

Requires the Department of Health to adopt culturally and linguistically appropriate procedures for offering the screening test and to provide standard hepatitis C information sheets to health care practitioners.

Requires the State Surgeon General to submit a status report to the Governor and Legislature by January 1, 2017, evaluating the effectiveness of the testing program.
