Voters who cast provisional ballots without signing the Provisional Ballot Voter's Certificate and Affirmation may cure this defect by submitting an affidavit with identification to the supervisor of elections by 5 p.m. on the second day following the election

The affidavit requires voters to swear they are qualified voters, have not voted more than once, and acknowledge that voting fraud is a third-degree felony punishable by up to $5,000 fine and 5 years imprisonment

Acceptable identification includes photo ID (passport, military ID, student ID, credit/debit card) or documents showing name and address (utility bill, bank statement, government check, paycheck)

Affidavits may be submitted by mail, personal delivery, fax, or email with attached identification copies

The Department of State and county supervisors of elections must post the affidavit form and instructions on their websites, including contact information for all supervisors
