Creates a statewide electronic database and clearinghouse for compassionate and palliative care plans, including POLST forms and advance directives, managed by the Agency for Health Care Administration and accessible to authorized health care providers.

Establishes physician orders for life-sustaining treatment (POLST) forms as a new standardized medical order document for patients with end-stage conditions or terminal illnesses likely to result in death within 1 year, requiring physician and patient signatures.

Authorizes hospital personnel, hospice staff, home health agencies, emergency medical technicians, paramedics, assisted living facilities, and other health care providers to withhold or withdraw cardiopulmonary resuscitation based on POLST forms, with immunity from criminal and civil liability.

Specifies POLST form requirements including patient directives on life-sustaining treatment and comfort care, procedures for minors requiring two additional provider certifications, methods for revocation, and provisions for out-of-state POLST forms to be recognized in Florida.

Prohibits facilities from requiring POLST forms as a condition of treatment, ensures POLST forms do not affect insurance policies or premiums, and establishes procedures for handling conflicts between POLST forms and advance directives based on most recent execution date.
