Extends sovereign immunity to employees or agents of health care providers that execute contracts with governmental contractors to deliver services to low-income recipients.

Requires health care providers and their agents to remain agents of the governmental contractor for 30 days after a patient is found ineligible, allowing continued treatment until transition to another provider.

Mandates written notice to patients or their legal representatives acknowledging that the provider is an agent of the governmental contractor and that the exclusive remedy for injury is through section 768.28 proceedings, with acknowledgment required at the initial visit.

Allows dental providers acting as agents under these contracts to accept voluntary monetary contributions from patients to cover costs of dental laboratory work, not to exceed actual costs.

Takes effect July 1, 2016.
