Allows voters who cast unsigned provisional ballots to submit an affidavit to cure the missing signature until 5 p.m. on the second day following the election, provided the supervisor can verify the voter's identity through information on the Provisional Ballot Voter's Certificate and Affirmation.

Requires the affidavit to include the voter's signature, address, and sworn statement that they voted only one provisional ballot and did not commit voting fraud, with penalties of up to $5,000 fine and 5 years imprisonment for violations.

Mandates instructions accompany the affidavit, allowing voters to submit it by mail, delivery, fax, or email along with a copy of valid identification such as a passport, driver's license, utility bill, or government document.

Requires the Department of Elections and each supervisor of elections to post the affidavit form and instructions on their websites, including the supervisor's mailing address, email address, and facsimile number.

Directs the supervisor of elections to attach received affidavits to the corresponding provisional ballot envelopes for canvassing board review when determining voter eligibility.
