Requires immunization records to be on file with Florida SHOTS (State Health Online Tracking System) before the 2017-2018 school year for all students in kindergarten through grade 12.

Mandates parents or guardians review an informational video prepared by the Department of Health and Florida Chapter of the American Academy of Pediatrics before receiving religious or medical exemptions from immunizations.

Establishes religious and medical exemptions as valid for only 2 years, requiring renewal after that period; adds permanent exemptions for specific immunodeficiency conditions to be determined by rule.

Requires the Department of Health to publish immunization and exemption rates for each public and private school by November 1 of each year, and requires schools with websites to post these rates on their sites.

Takes effect July 1, 2016, and directs the Department of Health to adopt rules implementing these immunization tracking and exemption requirements.
