Requires managed care plans, health insurers, and health maintenance organizations to cover prescription contraceptives and related outpatient consultations, examinations, procedures, and medical services necessary to prescribe, dispense, deliver, distribute, administer, or remove contraceptives.

Defines "prescription contraceptive" as a drug or device requiring a prescription and approved by the FDA to prevent pregnancy.

Permits coverage to be subject to the same copayments, deductibles, and coinsurance applied to other prescription drugs, and requires reimbursement for contraceptives intended to last 3 months upon first dispensing and 12 months for subsequent dispensings.

Exempts health insurance policies and health maintenance contracts offered by faith-based employers from contraceptive coverage requirements; defines "faith-based employer" as a nonprofit organization whose purpose is inculcating religious values and that primarily employs and serves persons sharing the employer's religious tenets.

Effective July 1, 2016.
