Amends section 408.910, Florida Statutes to make permanent the public records exemptions for the Florida Health Choices Program by removing the sunset repeal provision that was scheduled for October 2, 2016.

Exempts from public disclosure personal identifying information of enrollees or participants in the Florida Health Choices Program.

Exempts from public disclosure client and customer lists of buyer's representatives held by Florida Health Choices, Inc., and proprietary confidential business information held by the corporation.

Allows authorized disclosure of exempted information to other governmental entities performing official duties, to individuals with written consent from program applicants, and to the Florida Kidcare program for administration purposes.

Establishes a second-degree misdemeanor penalty for knowingly and willfully violating the exemption provisions.
