Creates a statewide wait list for enrollment in Medicaid long-term care managed care home and community-based services, managed by the Department of Elderly Affairs.

Requires prioritization of wait list applicants using a frailty-based screening tool that generates priority scores, with ties broken by earliest placement date on the wait list.

Mandates Aging Resource Center personnel conduct initial screenings and annual rescreenings (or upon significant changes in circumstances) before individuals can be placed on or remain on the wait list.

Establishes priority enrollment bypassing the wait list and screening process for three groups: individuals ages 18-20 with chronic debilitating diseases, nursing facility residents transitioning to community after 60+ consecutive days, and high-risk individuals referred by adult protective services.

Allows the Department of Elderly Affairs to terminate individuals from the wait list if they lack a current priority score, request removal, fail to respond to three documented contact attempts, receive an enrollment offer, or begin receiving services.
