Establishes the "Heartland Headwaters Protection and Sustainability Act" to protect water resources in the Green Swamp area and headwaters of six major Florida rivers (Alafia, Hillsborough, Kissimmee, Ocklawaha, Peace, and Withlacoochee).

Requires the Polk Regional Water Cooperative to prepare a comprehensive annual report listing water resource projects for state funding consideration, organized by category (drinking water, wastewater, stormwater/flood control, environmental restoration, and conservation).

Mandates the Polk Regional Water Cooperative submit its annual report by December 1, 2017, and each year thereafter to the Governor, President of the Senate, Speaker of the House, Department of Environmental Protection, and appropriate water management districts.

Requires the annual report to include priority rankings, estimated project costs, completion dates, and sources of financial assistance for each listed project.

Takes effect July 1, 2017.
