Amends chapter 2012-258, Laws of Florida to change the expiration date of the North Lake County Hospital District from 2027 to 2017 at the end of its fiscal year without further legislative action.

Allows the district to continue for 10-year periods if voters approve continuation in a referendum held during the general election.

Requires a referendum to be held on November 8, 2016 (or other general election date as provided by law) asking voters whether to continue the district for another 10 years.

Specifies that if the district is dissolved, all property owned by the district transfers to the Lake County Board of County Commissioners, which assumes all district indebtedness.

The referendum question addresses the district's authority to levy an ad valorem tax not exceeding 1 mill annually to fund indigent care for qualified district residents.
