Allows electors to submit an affidavit to cure vote-by-mail ballots that lack a signature or have a signature discrepancy with registration records.

Extends the affidavit submission deadline to 5 p.m. on the day before an election (changed from 2 days before).

Requires supervisors of elections to provide instructions with the affidavit that specify identification requirements and submission methods (mail, fax, or email).

Specifies acceptable forms of identification to accompany the affidavit, including photo identification or documents showing name and current residence address.

Effective date is July 1, 2017.
