Authorizes individuals to donate electronic health records and qualified electronic health records to approved medical/dental schools, universities, hospitals, or repositories for medical research and education purposes.

Establishes multiple methods for health records donation including signed donor cards, driver licenses/identification cards, living wills, wills, and witnessed documents using a specified Uniform Electronic Health Records Donor Card form.

Requires health records donated by health care surrogates or proxies to be de-identified unless the recipient provides written consent to include identifying information.

Directs the Agency for Health Care Administration and Department of Highway Safety and Motor Vehicles to develop a program allowing individuals to indicate intent to donate electronic health records when issuing or renewing driver licenses and identification cards, including notation on the cards and a website link to health records repositories.

Allows donors to amend or revoke health records donations through signed statements, oral statements to witnesses, documents found on the donor's person, or destruction of the donation document; effective July 1, 2017.
