Changes "prior to" to "before" in requirements for medical evaluations before student athletes participate in interscholastic sports or practices.

Specifies that medical evaluations must be administered by practitioners licensed under chapter 458, 459, or 460, or certified under s. 464.012, and in good standing with their professional regulatory board.

Requires the preparticipation physical evaluation form to include information advising students to complete a cardiovascular assessment, including an electrocardiogram, and to provide information about alternative cardiovascular evaluation and diagnostic tests.

Requires the form to include a place for the practitioner to attest that each examination procedure was performed by the practitioner or under their direct supervision, and to indicate if referrals to other practitioners were made.

Effective date is July 1, 2017.
