Requires health care practitioners licensed under chapters 458, 459, and 464 to submit and update vaccination data to the state immunization registry for children ages birth to 18 and college/university students ages 19-23 at Florida facilities, unless parents or students opt out.

Establishes electronic availability of immunization records from the registry to schools, child care facilities, and other entities required by law to obtain proof of immunizations.

Allows parents or guardians to refuse inclusion of their child in the immunization registry by signing a form, and permits college/university students ages 19-23 to opt out of registry participation.

Requires schools to have students' immunization certificates on file with either the state registry of immunizations or with the school prior to attendance in kindergarten through grade 12.

Effective date is January 1, 2020.
