Exempts the City of Marco Island from needing a certificate of public convenience and necessity from Collier County if the city's application is unapproved and other prerequisites are met.

Requires a third-party financial analysis of the city's assumption of full emergency medical service transport services at the paramedic level.

Authorizes the Department of Health to issue a license to Marco Island's Fire Rescue Department to provide prehospital, interfacility advanced life support, or basic life support transportation services upon meeting all criteria.

Requires approval by a majority vote of Marco Island qualified electors in a referendum held in conjunction with the primary election on August 28, 2018.

Takes effect upon voter approval in the August 28, 2018 referendum, except the referendum section takes effect upon becoming law.
