Deletes the requirement for physicians, clinics, and other medical providers to obtain a signed disclosure and acknowledgment form from insured patients at initial treatment to claim personal injury protection benefits.

Retains the requirement for providers to maintain a signed patient log for subsequent treatments, which hospitals can satisfy through their existing medical records system.

Exempts emergency department services and ambulance transport from the disclosure and acknowledgment form requirement.

Updates cross-references in s. 400.9905 and s. 627.7401 to conform with changes and adds notice about potential billing error reward provisions to insured notification forms.

Takes effect July 1, 2018.
