Requires the Department of Law Enforcement to include questions about a buyer's criminal history and firearm eligibility on the standard firearm purchase form.

Mandates the Department of Law Enforcement notify federal and state correctional, law enforcement, prosecutorial, and criminal justice agencies within 1 week when a potential firearm buyer receives a nonapproval number due to being prohibited from firearm possession.

Requires notification to include the buyer's identity, licensee identity, date and time of nonapproval, prohibiting criteria, and purchase location; notification may be delayed if an ongoing investigation would be compromised.

Allows the Department to identify appropriate agencies in each county to receive notifications and to deliver notifications through oral, written, or electronic means.
