Allows insurers to add new prescription drugs to covered drug lists at any time during the policy year.

Requires small employer carriers to comply with the same prescription drug formulary restrictions as other insurers under health benefit plans.

Applies restrictions to health maintenance organizations' health maintenance contracts, with exceptions for grandfathered health plans and specified mandatory benefits; effective January 1, 2019.
