Creates s. 382.0155, F.S., allowing the Department of Health to issue noncertified copies of original birth certificates to adoptees and certain relatives under specified conditions.

For adoptions finalized on or after July 1, 2018, grants immediate access to adoptees age 18+, adult descendants with adoptee consent or death certificate, and birth parents named on the original certificate.

For adoptions finalized between July 1, 1977 and July 1, 2018, beginning February 1, 2019, requires both birth parent and adoptee/descendant to request access, with additional conditions including 40-year finalization minimum or presumed death of birth parent.

Requires the Department of Health to develop and maintain a contact preference and medical history form for birth parents, allowing them to indicate contact preferences and provide medical information that may be released with their authorization.

Directs the Department of Health to maintain and publish on its website statistics on contact preference forms filed, broken down by contact, indirect contact, or no-contact preferences.
