Establishes the "Patient's Freedom of Choice of Providers Act" to prohibit health insurers from excluding willing and qualified health care providers from their networks based on patient choice protections.

Applies to health care providers licensed under specified Florida Statutes chapters covering physicians, dentists, nurses, pharmacists, mental health professionals, and other licensed practitioners.

Applies to health insurance plans offered by HMOs, health insurers, and ERISA-covered employee welfare benefit plans, excluding limited benefit, specified disease, accident-only, Medicare supplement, dental, vision, long-term care, and workers' compensation plans.

Prohibits health insurers from obstructing patient choice by excluding licensed health care providers from participating in provider networks if the provider is located within the plan's geographic coverage area and willing to meet the insurer's terms, conditions, and fee schedules.

Takes effect July 1, 2018.
