Requires nursing homes to provide residents and the State Long-Term Care Ombudsman Program with current contact information including a functional telephone number, and notify them of any changes within 30 days.

Authorizes residents or their representatives to make record access requests orally (in addition to written requests) and requires facilities to provide access within 24 hours, excluding weekends and holidays, and deliver copies within 2 working days.

Limits psychiatric record disclosure refusals to situations where disclosure would reasonably endanger the life or physical safety of the resident or another person, rather than merely being detrimental to health.

Requires assisted living facilities to provide written notice of relocation or residency termination to residents and send a copy to the State Long-Term Care Ombudsman Program representative within 7 calendar days.

Makes failure to provide current contact information to residents and the ombudsman program grounds for agency action against a nursing home licensee.
