Allows adoptees age 18 and older (or their descendants if deceased) to request a noncertified copy of their original birth certificate from the Department of Health.

Requires the Department of Health to issue the noncertified copy within 90 days of receiving a compliant application.

Mandates the Department of Health create a contact preference form for birth parents to indicate whether they want to be contacted by adoptees, prefer intermediary contact only, or do not want contact.

Requires birth parents be provided a medical history form to complete and file with the Department of Health, with a HIPAA privacy disclaimer.

Authorizes the Department of Health to charge a fee for the noncertified copy not exceeding the fee for a certified copy and requires implementation policies and procedures by February 1, 2019.
