Requires the Agency for Health Care Administration to design annual patient-safety culture surveys for hospitals and ambulatory surgical centers to measure adverse events, teamwork, error reporting comfort, and staff compliance with safety regulations.

Mandates surveys be submitted anonymously to encourage participation and directs the agency to review nationally recognized survey products including those from the federal Agency for Healthcare Research and Quality and the University of Texas Safety Attitudes Questionnaire.

Requires hospitals and ambulatory surgical centers to submit patient-safety culture survey data to the agency as a condition of licensure under chapter 395.

Adds patient-safety culture survey data to the list of health care data that facilities must submit to the agency for transparency and quality measure purposes.

Appropriates $352,919 in recurring funds from the Health Care Trust Fund and authorizes one full-time equivalent position for fiscal year 2018-2019 to implement the requirements, with an effective date of July 1, 2018.
