Removes the provision allowing parents or guardians to refuse inclusion of their child in the immunization registry.

Requires health care practitioners licensed under chapters 458, 459, or 464 to report vaccination data to the immunization registry for children from birth to 18 years of age and for college/university students 18-23 years of age receiving vaccinations at student health facilities.

Changes immunization records from being "transferred to" to "electronically available to" schools and licensed child care facilities that are required by law to have such records.

Requires students to have a certificate of immunization on file with the state immunization registry (rather than with the school) prior to school attendance in grades kindergarten through 12.

Authorizes the Department of Health to adopt rules to implement the immunization registry provisions and takes effect January 1, 2020.
