Limits preview games or machines to 10 units at tax-exempt organizations, 5 units at bars or truck stops, and 3 units at beer or wine establishments; operators may only lease machines from licensed lessors.

Establishes annual licensing fees of $10,000 for manufacturers, $5,000 for lessors, $500 per operator location, and $125 per individual machine; requires licensed lessors to submit annual reports and quarterly change notifications.

Imposes criminal penalties including second-degree misdemeanor for first violations, first-degree misdemeanor for second violations, and third-degree felony for subsequent violations; restricts legal actions to government agencies or substantially affected residents within 5 miles of the location.
