Parents or guardians of children (birth through 17) and college students (18-23) receiving vaccinations at campus health centers may opt out of Florida's immunization registry by signing a form, which requires complete removal of their records and identifying information from the registry

Health care practitioners licensed under chapters 458, 459, or 464 who administer vaccinations to children (birth through 17) or college students (18-23) at campus health centers are required to report vaccination data to the immunization registry unless the individual has opted out

Consent to treatment forms for vaccinations administered to children must include notice that parents/guardians may refuse to have their child included in the immunization registry

Students entering Florida public or private schools (K-12) must have immunization certification on file with the Department of Health's immunization registry, or present certification directly to the school if they have opted out of the registry

District school boards and private school governing authorities must establish policies requiring age-appropriate scoliosis screening for students; effective date January 1, 2021
