Requires health care practitioners (physicians, osteopaths, nurses) to report vaccination data to the state immunization registry for children from birth through 18 years old and college students ages 19-23 receiving vaccines at Florida university health centers

Allows parents/guardians to opt their children out of the immunization registry by signing a form; if opted out, all records must be removed and no reference to the child may remain in the registry

Permits college and university students ages 19-23 who receive vaccinations at Florida campus health centers to opt out of the immunization registry

Requires consent to treatment forms for childhood vaccinations to include notice that parents may refuse to have their child included in the registry

Changes school-entry requirements so students must have immunization certificates on file with the Department of Health's immunization registry rather than directly with schools (except for children opted out of the registry)

Effective date: January 1, 2021
