Authorizes the Agency for Health Care Administration to pay for donor human milk as an optional Medicaid service for both home and inpatient use when ordered by a licensed physician or nurse practitioner

Establishes eligibility criteria for infants: birth weight of 1,500 grams or less, congenital or acquired intestinal condition with high risk for feeding intolerance, necrotizing enterocolitis, or infection, or medical inability to receive maternal breast milk

Requires donor human milk to be procured from a nonprofit milk bank certified by the Human Milk Banking Association of North America (HMBANA)

Adds donor human milk bank services to the list of Medicaid services reimbursable on a fee-for-service basis and requires managed care plans to include it as a minimum covered benefit

Sets reimbursement at not less than the reasonable cost of milk from an HMBANA-certified bank plus reasonable processing and handling fees, effective July 1, 2019
