Prohibits the Department of Health from establishing minimum or maximum amounts for Closing the Gap grants and requires award amounts to be determined based on individual application merits.

Requires the department to ensure grants are awarded to applicants in various regions of the state.

Removes provisions dedicating up to 20 percent of Closing the Gap grant program funding to Front Porch Florida Communities projects.

Adds Alzheimer's disease and dementia as a priority area that may be addressed in Closing the Gap grant proposals, bringing the total priority areas to 11.

Takes effect July 1, 2019.
