Expands definition of "extended family member" to include individuals who qualify as "fictive kin" under section 39.01, in addition to blood relatives within the third degree and current stepparents.

Modifies concurrent custody requirements to allow individuals to seek custody if they currently have or have had physical custody of the child for at least 10 days in any 30-day period within the last 12 months.

Authorizes courts to approve provisions in temporary or concurrent custody orders related to the best interest of the child, including reasonable transition plans for returning custody to parents.

Allows courts to order on their own motion the transitioning of a child back to parental custody when the child has been in temporary custody for a significant period after finding parents unfit, considering factors such as length of placement, child's developmental stage, and time needed for transition.

Permits courts to require parties to comply with approved transitioning provisions before terminating temporary or concurrent custody orders.
