Reclassifies violation of requirements for infants born alive from a first-degree misdemeanor to a third-degree felony, with penalties under sections 775.082, 775.083, or 775.084.

Changes the "Parental Notice of Abortion Act" to the "Parental Notice of and Consent for Abortion Act" and requires physicians to obtain written parental or legal guardian consent before performing abortion on a minor.

Requires the consenting parent or legal guardian to provide government-issued photo identification and a notarized, dated, and initialed document containing a specific consent statement, with copies retained in medical records for 5-7 years.

Establishes exceptions to parental consent requirements for medical emergencies, court-ordered waivers, and certain waived notifications, and makes intentional or reckless violation of consent requirements a third-degree felony.

Effective date is July 1, 2020.
