Requires nursing homes implementing formularies to establish a committee consisting of the medical director, director of nursing services, and a certified consultant pharmacist to develop the formulary and related guidelines

Mandates quarterly monitoring of compliance with formulary policies and tracking of clinical outcomes when therapeutic substitutions occur

Requires prescribers to annually authorize use of the institutional formulary for their patients and opt into any subsequent changes; prescribers may opt out for specific patients, drugs, or drug classes

Allows prescribers to prevent therapeutic substitution on individual prescriptions by indicating "NO THERAPEUTIC SUBSTITUTION" and prohibits nursing homes from taking adverse action against prescribers who refuse to participate in the formulary system
