Creates the Medicaid Pharmacy Benefit Savings Optimization Act requiring the Agency for Health Care Administration to select a single pharmacy benefit administrator through competitive procurement to manage all pharmacy benefits for Medicaid managed care recipients.

Requires completion of the procurement process and administrator selection by October 1, 2021, after which managed care organizations are prohibited from providing pharmacy benefits to their members.

Mandates the pharmacy benefit administrator contract include fiduciary duty to the state, pass-through pricing, use of the state's existing preferred drug list and reimbursement methodologies, prohibition on mail-order pharmacy requirements, and prohibition on excluding willing pharmacies from the network.

Requires the Agency to calculate 20 percent of each managed care organization's net underwriting gain for the July 1, 2020 through June 30, 2021 contract year and reduce subsequent contract payments by that amount to fund pharmacy benefits and implementation costs.
