Establishes telehealth minority maternity care pilot programs in Duval County and Orange County by July 1, 2022, to expand maternal health services for racial and ethnic minority populations using technology-enabled remote care models.

Requires pilot programs to provide eligible pregnant women telehealth services addressing pregnancy complications (anxiety, depression, substance use disorder, hemorrhage, infection, hypertensive disorders, diabetes, cardiovascular conditions) and social determinants of health (housing, transportation, nutrition, childcare, wellness programs).

Mandates training for participating health care practitioners and perinatal professionals on implicit bias, racism, discrimination in maternity care, remote patient monitoring, screening for social determinants of health, and maternal mental health and substance use disorder treatment.

Adds severe maternal morbidity and other maternal health outcomes as priority areas under the Closing the Gap grant program for addressing racial and ethnic health disparities.

Funds pilot programs through existing Closing the Gap grant appropriations and authorizes the Department of Health's Division of Community Health Promotion and Office of Minority Health and Health Equity to apply for additional federal funding.
