• Beginning January 1, 2022, health insurers and pharmacy benefit managers must establish a secure, interactive online prior authorization process for prescription drugs and medical services that allows documentation uploads.

• All prior authorization requests from health care providers must be submitted through the online process starting January 1, 2022; the process may not require unnecessary information unrelated to medical necessity determinations.

• Health insurers and pharmacy benefit managers must make current prior authorization requirements readily accessible on their websites and provide 60 days' advance notice before implementing new requirements or restrictions.

• Prior authorization decisions must be made within 3 calendar days for non-urgent services and within 24 hours for urgent health care services after obtaining all necessary information.

• Health insurers and health maintenance organizations may not impose additional prior authorization requirements for surgical or invasive procedures performed during the perioperative period of another procedure that was already authorized.
