Creates Prevention of Maternal Mortality Grant Program requiring Department of Health to develop and implement by July 1, 2022, awarding grants to community-based organizations serving black women with evidence-based maternal health programs.

Establishes grants for training programs that reduce bias, racism, and discrimination in maternity care settings, with special consideration for programs providing periodic training to all birthing professionals and staff.

Requires Department of Health to award grants by July 1, 2022, to eligible entities for expanding technology-enabled collaborative learning models and improving maternal health outcomes in health professional shortage areas and medically underserved populations.

Creates Investments in Digital Tools to Promote Equity in Maternal Health Outcomes Program by July 1, 2022, to award grants for increasing access to digital tools related to maternal health care to reduce racial and ethnic disparities.

Requires Department of Health to submit annual reports by July 1, 2023, and thereafter to Governor and Legislature evaluating grant program effectiveness, best practices, and recommendations for future maternal health initiatives.
