Prohibits business entities and governmental entities from requiring COVID-19 vaccination documentation, proof of post-infection recovery, or COVID-19 tests as conditions of employment, service access, or contracting.

Prohibits business and governmental entities from discriminating in hiring, employment, or service provision based on a person's vaccination status, COVID-19 recovery status, or refusal to take COVID-19 tests.

Prohibits business entities, governmental entities, and educational institutions from requiring face masks or facial coverings and from discriminating based on refusal to wear such coverings, with exceptions for health care settings and occupational/laboratory safety requirements.

Establishes that health care practitioners must obtain informed consent from COVID-19 patients before prescribing treatment medications and must discuss available alternatives, including FDA-authorized medications.

Creates administrative penalties of up to $5,000 per violation enforceable by the Department of Health and Department of Legal Affairs, with collected fines deposited in the General Revenue Fund.
