Requires individual, group, blanket, and franchise health insurance policies and health maintenance contracts issued or renewed on or after July 1, 2025, to cover one mammogram each year (including digital breast tomosynthesis) for women ages 25–40, replacing the prior age-tiered schedule that began baseline screening at age 35

Mandates coverage of one annual supplemental breast cancer screening (e.g., MRI, ultrasound, molecular breast imaging) for women at increased risk due to dense breast tissue, personal or family history of breast cancer, biopsy-proven benign breast disease, ancestry, genetic predisposition, not having given birth before age 30, or other physician-determined reasons

Creates s. 409.9064, F.S., requiring the Agency for Health Care Administration to provide yearly mammogram and supplemental breast cancer screening coverage for female Medicaid recipients ages 25–40, subject to fund availability and General Appropriations Act limitations, and to seek federal approval if needed

Removes prior physician prescription requirements for mammogram coverage and eliminates the previous age-based frequency distinctions (every 2 years for ages 40–49, annually for 50+), consolidating the mandate around annual coverage for women 25–40

Effective date is July 1, 2025
