Establishes the "Bridging Systems to Housing Act" requiring the Department of Children and Families to conduct a pilot program in Broward, Duval, Hillsborough, and Pinellas Counties for discharge procedures at hospitals and inpatient medical facilities

Requires facilities to develop early assessment processes for identifying persons who are homeless at intake, at risk of homelessness, or face health/safety threats upon discharge, with discharge plans connecting them to the local Continuum of Care

Mandates facilities enroll in the Homeless Management Information System to share client-level data on housing and service needs, or develop alternative data-sharing mechanisms with client consent

Requires the Department of Children and Families to submit quarterly progress reports during implementation and annual effectiveness reports to the Governor and Legislature beginning November 30, 2027

Effective July 1, 2026; procedures apply to consenting individuals housed more than 24 hours, with no cause of action created against the state or facilities for failure to follow the guidelines
