Creates a demonstration pilot program for "de-risking disease behavioral health intervention services" in Medicaid Region G (Indian River, Martin, Okeechobee, Palm Beach, and St. Lucie Counties), contingent upon appropriation

Targets 1,000 Medicaid recipients diagnosed with both a behavioral health condition (depression or anxiety) and a chronic disease (diabetes mellitus, obesity, or coronary heart disease)

Provides five concurrent services: behavioral health counseling from licensed clinical social workers, medical nutrition therapy from registered dietitians, biomarker health tracking devices, disease nutrition education, and medical nutrition supports including medically tailored meals and produce prescriptions

Requires program design and evaluation by Tufts University Friedman School of Nutrition Science and Policy and the Florida Health and Nutrition Coalition, with data collection on participant compliance, health outcomes, and biomarkers

Mandates a report to the Legislature after program expiration with usage data, collected findings, and legislative recommendations; pilot program expires August 31, 2030, and the entire section expires September 1, 2031
