Requires health insurance policies issued or renewed on or after January 1, 2027, to cover annual screening mammograms for women age 40 and older, eliminating the previous baseline mammogram requirement for ages 35-39 and biennial screening for ages 40-49

Mandates coverage for additional medically necessary screening mammograms and one supplemental breast cancer screening per year (including MRI, ultrasound, or molecular breast imaging) for women at "increased risk" of developing breast cancer

Defines "increased risk" to include genetic predisposition, lifetime breast cancer risk of 20% or greater, prior thoracic radiation between ages 10-30, heterogeneously or extremely dense breast tissue, and history of certain precancerous conditions

Applies coverage requirements to women with dense breast tissue and to breast cancer survivors in remission and surveillance periods, provided examinations do not meet the diagnostic breast examination definition

Requires insurers to offer coverage without deductible or coinsurance provisions for an appropriate additional premium, and mandates compliance with National Comprehensive Cancer Network Breast Cancer Screening and Diagnosis guidelines
