Mandates coverage for at least three complete egg retrievals with unlimited embryo transfers, following American Society for Reproductive Medicine guidelines

Prohibits insurers from imposing waiting periods, using prior fertility diagnoses to limit coverage, or restricting coverage based on use of donor gametes, donor embryos, or surrogacy

Covers fertility preservation for individuals with medical conditions or undergoing treatments that may impair fertility, including cryopreservation storage for three years

Excludes coverage for experimental fertility procedures and nonmedical costs related to donors or surrogacy
