Creates a voluntary "do not sell" firearms registry allowing individuals to self-prohibit from purchasing firearms by registering in person with a healthcare provider or probate court judge

Requires registrants to provide contact information for two individuals who will be notified if the person later requests removal from the list

Establishes a mandatory 7-day waiting period before requesting removal and a 21-day processing period for removal requests, after which all related records must be destroyed

Makes selling a firearm to someone known to be on the list a felony, while possessing a firearm while on the list carries a $25 civil penalty

Prohibits use of registration or removal evidence in court proceedings, employment decisions, insurance determinations, housing discrimination, government benefits, or healthcare service denial
